Case Manager

About the Role

Residential Youth Care’s Case Manager is responsible for providing support and linkage to adolescents in residential treatment programs and RYC services, and their families. Case managers are responsible for documentation and case coordination/ maintenance. They are an integral part of our client's treatment teams and work towards  RYC's mission to advocate for and foster the safety and well-being of youth at risk and their families so they may lead productive lives.

  • Excellent written and verbal communication skills

  • Exceptional organizational skills, ability to multi-task in a fast paced environment

  • Ability to work well as a team and independently, as job duties require

  • Ability to implement coping strategies and conflict resolution skills in potentially volatile situations


Primary Responsibilities:

  1. Provide case management services to clients and families of RYC, including but not limited to coordinating the delivery of individualized treatment & services, scheduling appointments, meetings, communicating (written & verbal) with treatment team members, facilitating treatment team meetings, providing general linkage and support

  2. Participate in the development of treatment plans based on problem areas identified in behavioral health assessments , and facilitate the development of the treatment goals with client and directing clinician

  3. Maintain and update all required paperwork and documentation, including but not limited to client note templates, TX plan reviews, status change forms, transition and discharge summaries,

  4. Maintenance of internal and external databases through regular data entry and updating

  5. Attend and participate in required agency meetings, effectively communicating relevant information

  6. Abide by federal and state regulations, and adhere strictly to agency policy and procedures to meet or exceed licensing, auditing, and accreditation standards

  7. Maintain strict confidentiality and uphold professional ethics in all practices and procedures

  8. Other job duties to be assigned


Requirements

  • High School Diploma or GED

  • 21 years of age+

  • Valid Alaska Drivers License and clean driving record

  • Basic knowledge of office equipment and computers, including word processing and email

  • Must be able to obtain First Aid/CPR and MANDT certification following hire

  • Must be able to pass a pre-employment background check and drug screening

  • Experience in Behavioral Health, Social Services, or working with Youth (preferred)

Job Type: Full Time, Non-Exempt

Hours Per Week: 40

$20.00-25.00/hr DOE
